ChloSite – Controlled Release, Chlorhexidine-Containing Bioadhesive Gel

Chlo-Site is a xanthan-based, dual-form chlorhexidine bioadhesive gel designed for subgingival application in periodontal therapy. Developed for long-lasting, controlled release, it is used as an adjunct to mechanical debridement (SRP) in the treatment of periodontal and peri-implant conditions.

Thanks to its gel structure, Chlo-Site adheres to soft tissues and remains within the pocket for up to 20 days, effectively suppressing bacterial biofilm and preventing recolonization.

💊 Pharmacological Composition

Chlo-Site contains a patented dual chlorhexidine system for both immediate and prolonged antibacterial action:

  • Chlorhexidine Digluconate 0.5% – rapid antimicrobial effect

  • Chlorhexidine Dihydrochloride 1.0% – long-lasting controlled release

The xanthan gum carrier provides a dense, viscoelastic matrix that retains active ingredients in the periodontal pocket, ensuring sustained release and strong mucoadhesion against oral fluids.

⚙️ Mechanism of Action

  • Disrupts bacterial membranes, causing microbial death

  • Creates a locally high chlorhexidine concentration in the subgingival area

  • Xanthan polymer ensures slow, continuous release of active components

  • Effective for up to 20 days, preventing recolonization during healing

🦷 Clinical Application Areas

Chlo-Site is recommended as a local treatment adjunct in:

  • Gingivitis and early stage periodontitis (Stage I–II)

  • Deep periodontal pockets (≥5 mm) – supportive therapy in Stage III–IV

  • Treatment of peri-implantitis and mucositis

  • Management of residual pockets after SRP

  • Supportive maintenance around dental implants

  • Alternative to non-surgical periodontal therapy

💉 Clinical Application Method

  1. Perform mechanical debridement (SRP) before application

  2. Apply the gel directly into the periodontal or peri-implant pocket using the syringe

  3. Formats:

    • 1 ml syringe – for multiple sites

    • 0.25 ml syringe – for localized lesions

  4. Post-care: avoid rinsing for 2 hours and refrain from hard chewing

  5. Applications may be repeated every 1–5 weeks, depending on healing

📊 Clinical Data

In multicenter randomized controlled studies, Chlo-Site demonstrated:

  • 0.8–0.9 mm reduction in probing depth

  • Significant decrease in P. gingivalis and T. forsythia levels

  • Antibacterial effect sustained up to 6 months

  • SRP + Chlo-Site achieved superior outcomes compared to SRP alone (p < 0.001)

📦 Forms and Packaging

  • 1 ml sterile syringe – suitable for multiple pocket applications

✅ Advantages

  • Strong local effect without systemic antibiotics

  • Accelerates healing when applied after SRP

  • The only gel formulation active for 20 days

  • High tissue compatibility, minimal irritation risk

  • Can be applied by dentists or dental hygienists

🩺 Application Protocols by Indication

  1. Mucositis & Gingivitis

    • Clean and dry the area before application

    • Apply directly to the lesion site

    • Reapply at the next visit if symptoms persist

    • Follow-up intervals guided by PSR values

  2. Stage I Periodontitis / Pockets < 3 mm

    • Apply after supragingival plaque removal

    • Plan at least 2 repeat applications

    • Adjust frequency per clinician’s judgment

  3. Stage II Periodontitis / 4 mm Pockets

    • Apply after SRP

    • 3 or more sessions recommended after initial use

    • Additional sessions based on healing progress

  4. Stage III Periodontitis / Pockets ≥ 5 mm

    • Apply after SRP

    • At least 4 applications required, with ongoing follow-up

  5. Stage IV Periodontitis / Pockets ≥ 6 mm

    • Apply after intensive SRP

    • At least 5 applications advised, continued use based on progression

  6. Protective Use in Stable Periodontal Patients

    • For reduced periodontium or borderline probing depths

    • Apply after supragingival cleaning during annual check-ups

    • Helps maintain long-term periodontal stability

  7. Prosthetic Implantology

    • Apply to gaps inside implants (fixture level or prosthetic parts such as MUA)

    • Supports peri-implant tissue health

  8. Pre-Periodontal Surgery Preparation

    • Apply to sulcus/pockets after supragingival or juxtagingival cleaning

    • Reduces microbial load in the surgical site

    • Improves tissue quality and healing outcomes
